Contact Points for Submission of the Applications in the Capital City of Prague

(with residence in Prague, including Prague-East district and Prague-West district)
We would like to highlight that on Prague MOI offices it is possible to submit your application about a registration certificate for an EU citizen by postal service.
Within improving the service of MOI office, reducing the number of client visits and increasing comfort you can only fill in this form (pdf, 6 MB) and send it with all requirements to address of relevant MOI office due to your residence (we recommend you to send it by registered post):

In case that this submission contains completed form and all requirements *, you will be contacted by administrative authority to check your passport and to take your certificate of temporary residence for an EU citizen. You will be contacted on the specific soonest term.
* The requirements for the application have to be made out in the Czech language or Slovak language or officially translated into Czech. All of the presented documents have to be submitted in originals or notarized copies. The notarized copies o public documents issued by the Member States' authority to citizens of this Member State in accordance to the Regulation (EU) 2016/1191 of the European Parliament and the Council will be accepted without any further verification (apostile, superlegalization). In the case of this submission it is possible to send a simple copy of the travel document and to submit its original personally at the MOI office before an issuing the Certificate of Temporary Residence of an EU citizen.
